Fungal infections tend to rarely happen and can be mistaken for columnaris. Could you please get a clearer pic?

Columnaris (Flexibacter columnaris)
Synonyms:
Cotton-wool, Mouth Fungus, Flexibacter

Symptoms:
White mouth with tiny filaments, white areas near edge of scales, clamped fins, fins start to rot in the later stage, fish struggles and sways near the surface

Causes:
This is bacterial infection which is very common in the aquaria and is very contagious. Immediate action is needed to contain it before it spreads to other fish.

Treatment:
Broad spectrum antibiotics, Sera Baktopur, Binox, Maracyn I & II, Kanacyn, Terramycin, Acriflavine, Furan, Copper Sulfate

Flexibacter columnaris thrives very well in warm waters therefore it is necessary to gradually lower the temperature to prevent it from progressing.

Body Fungus (Saprolegnia spp.)
Symptoms:
White cottony patches on the skin with long filaments that stand away

Symptoms must be differentiated from the false mouth fungus, Columnaris. It must be noted that fungus happens less compared to columnaris.

Causes:
Fungi are decomposing organisms that exist in every aquarium. The wound must always be treated immediately. If left untreated, fungal spores can attach themselves on the wounds and eventually harm the fish. They serve as a secondary infection to the wounds and open sores.

Treatment:
Methylene Blue, Jungle Fungus Guard, Mardel Maroxy

here some more pics im almost certain its columnaris with a secondary fungal infection Its getting worse so i ceased using the tetracycline and now using macryn 1,2 and maroxy for fungal Im hoping he can be cured.

I did a 80% wc before I added the meds and ideally would have liked an empty tank but this is all i got for now Im hoping the decaying plant matter wont make it worse

That looks rough but IMO it looks like a cut with some fungus on it. I think Pimafix or Meth Blue should take care of this. Let us know how the situation progresses, hope he pulls through.

my pictus cats had that after they were bleeding from all over from lack of oxygen, i had to put them down

Oxygen deprivation can cause bleeding? Well there are a ton of plants in that tank he's in now, right?

How can you tell these two apart?

yea tons of plants in that tank plus airstones and yeah I cant distinguish columnaris from reg fungus so Im just treating for both think it got as worse as it will hopefully its starting to heal

Fungus happens rarely. It will occur when wounds are left untreated allowing the fungus spores to establish there and develop in white to gray cottony patches. I've already included the symptoms of both previously. Mixed infection is possible.
