We're going to have to see what it looks like to tell you about the spot on the lip. Sounds like it is just a sore. I had a pleco that had something similar, it went away on its own.
As for the feces, you might try to get in touch with Lupin, he would know as well as anyone.

Looks as though it is inflamed. It could be sore and bothering him, hence, why he isn't eating.
Have you added anything to the tank? Salt? Upped the temps?
If it was me..... a nice dose of Pimafix and Melafix would be in his future.
