There really doesn't appear to be any obvious indication of what happened so you may never know. But to put it in perspective this just happen all the time (there being no obvious indication), it just gets to you when its a beloved specimen.
I am sorry to hear about your loss. That was a beautiful fish, very sad indeed.
When you put your hands in the tank did you have anything on your hands? Soap residue, lotion, mosquito repellant? Anyting at all that could have caused a problem? I have a friend that forgot they sprayed mosquito repellant on and then hours later put thier hand in the tank and lost a few fish that way.
Major bummer...hate to see a pet just pass like that. So far I have gotten lucky with my new setup, everyone has stayed healthy, no deaths yet. In my last setup I had a few, always sad.
"Another reason for the death MIGHT be due through human interaction with the water. Meaning some chemical agent was accidentally introduced via perfume, deodorant, left over soap on your hands, etc. Just a thought....
Was it housed with anything else? How are the remaining fish? Have you tested your water?"

There are 2 5 inch Oscars, a 13 inch jardini and a 8 inch pleco. I always wash my hand and arms before I put them in the tank. I use a soap that isn't toxic to fish. The remaining fish are in great health.
