Yes, fairly quickly. But it's also very easy to treat with heat and salt.
 
Buy Morton's solar salt from Home Depot. A big bag for around 4 bucks or so. It is the same as aquarium salt, and it is way, way cheaper. Temp at 84-86 degrees or so, and add 3 tablespoons of the salt for every 5 gallons of tank volume.
 
Do it in the regular aquarium. If one fish in the tank has it, then they all do, even if you don't see it on the others yet. You want to kill all Ich present in the aquarium. Do the treatment I posted above, and don't do a water change until 3 days after the Ich disappears (this is assuming that your water parameters are in check and an earlier water change is not needed).
 
I have a really bad ick problem in my 10 gallon with my buenos aires and other tetras and I was wondering if my heater is hot enough because I dropped in some ick tablets every 24 hours and it turned blue on the sealant but now it is still cold in my tank I have a 100 watt heater I don't know what side is hot or cold but in my 55 I have a heater the same kind just bigger and that one is in the same direction as this and that tank is around 78 degrees always and the 10 gallon is only at like I'm guessing high 60s low 70s but it's cold and I need to get rid of this ick fast and nothing is working and my tank is too cold so I can't do the heat treatment but I'll post pics of the heater I got it used so I had no instructions of how to use it so I'm just guessing on this and in either direction it's still cold after 24 hours

If you haven't added the salt quite yet, I would suggest adding it gradually over a few hours, or even a day so you don't shock the fish.

Also, I think you are supposed to keep up the treatment for a full week or two after the last visible sign of ich because of their non-parasitic reproduction cycle. Maybe I'm wrong and someone can comment.
 
Andrew, not sure what you mean by direction. As long as the heater is in the water it should do its job. From the sounds of it, it seems you have a heater that no longer works - or the internal thermometer is way off and you have to adjust accordingly.

From what I have read, most medicines are not very effective while the heat and salt treatment is highly effective.
