Is this a leach? A worm?

The head looks wrong for a earthworm. Unless he is feeding earthworms, I doubt one would be in his/her aquarium. Leeches can be skinny looking and also have body segments

I'm not an expert, so it could be some other kind of aquatic worm

I's been months since I've fed earthworms, but that's not to say one didn't hide in the substrate and maybe get sucked up into the filter. I agree with banjocat... the head doesn't look like an earthworm, nor does it's movement.

Here's your answer if your fish is WC....

However, newly imported specimens may be suffering from an infection of a freshwater leech, Macrogyrodactylus polypteri that attaches itself to the body of the fish. The fish may try to seek relief by attempting to scratch itself, behaviour often referred to as “flashing” or “flicking”. Formalin baths are effective in removing this parasite.

i found one that looked like that and i put it in my tanks for food .
your leech is a leech to test put it on you hand it should suck on your hand and wont readily let go.
