that beak-like head is common in a lot of JDs from what I've seen, more specifically to low quality EBJDs. Usually they are culled early, but this guy looks rather large so he probably doesn't have any issues eating. That unfortunately doesn't look like something that can be healed Is he missing his left eye? It does look a little strange, but it could just be the angle of the photo. I'm sure he'll be 10x happier in a well cared for tank rather than an LFS (Y)
Yeah his left eye is flat with a small pupil. The right one works he has bad depth perception. But he eats worms, hilkari gold and algae disks. I got him in a 300 gallon right now with an Oscar and a blood parrot and a couple plecos
