I see 3 things wrong.....

-gill curl
-cut lips
-eyes aren't red

The shape looks of a rhombeus but it also appears to be extremely stressed.

Ok have don first water test:
Nitrite = 0
PH = 6
Ammonia= 0
Nitrate = 10-20ppm

Read up gill curl (thanks) will try combo of vitamins C and calcium both have been recommended with 25% water-change and salt.

From other reading on the eye colour it maybe either breed (I'll post some clearer pics) or it could be cloudy eye and the recommendations seem to be an increase in temp to 83C and salt and if no improvement after a week or so Melaflix.

General condition is probably diet as I was told to feed him steak (yes that I sorted now I've moved to white fish but I may try pellets to add vitamins)

The tank is a bit small for him he's 20cm tank is 60cm cube. He has been at my place now for 4 weeks.

Thanks for the link to the plants too. I was reading that earlier and would work well in the current location.

Yea that's what I thought but by its stressed appearance I couldn't 100% say.

This kind of stress doesn't happen within 4 weeks, this has been going on for year/s! Keep the water clean with biweekly 40% water changes and try pothos but definitely focus on clean water and a good diet of pellets/white fish fillets! A temp of 78-80°f is fine.

Thanks. I'll get some better photos loaded soon. Did the first major water change. Hauled out old slimy sea shells and moved some grasses (plastic) around to give him some more room- he has a cluple of little fish friends that need the grass). With tank full and these changes he has more space. Then spent 2 one hour sessions coaxing (now calling it training) to get him to swim the whole tank. Will see how he's going in the morning but by the end of tonight he was swimming full tank solo. I used a piece of red cloth. He's always responded to colour. So,i dAngled the cloth in front of him until he was following it around the tank. How Bout that bull fighting with a piranha! Hope I didn't add to the stress but I figured it would be an improvement on head butting the glass in one corner.

will retest water tomorrow am. I also noticed there is a lot of crap in the gravel. Should I get a vacuum thingy asap or remove gravel as much as possible and rinse out crap and replace? Or even remove gravel and replace with sand. Seems most PKeepers prefer sand (why?).
 
I don't the fish looks very stressed or anything, just the angle of the picture and shadowing made it look "unusual", the lips on mouth does look a bit strange tho. that's the only thing I notice being "weird" based on the original picture posted.
the gill curl part, it's very difficult to tell from the angle, much easier to see if look from behind or below so if there's any "curled" part can be easily spotted.
if you noticed that there's a lot of waste in the substrate then you should remove them other wise it will speed up the ammonia production and cause your water to go bad faster.
 
hi rescue fish still swimming. water tests good.

Here are some updated and hopefully clearer photos from all sides to help confirm ID. I had discounted Rhom de to head shape and Natterei because there is no red on him at all. Was starting to think might be some kind of Pygocentrus Piriya or Pygocentrus Ternezi as it has pale underbelly BUT not yellow.

Anyway I hope these additional photos can help someone with more than 4 week piranha experience to have a go on an ID confirmation
