I didn't shake the bottle before I put it in.. but i shook the test tube.. should I test again?
 
I think your water has low levels of dissolved oxygen. That's why your fishes are breathing rapidly or at the surface. What do you use to dissolve oxygen in your aquarium?

I was thinking that might be a possibility also....
I run an FX5 pointed at the surface and a HOB.. Last time I did a water change, I moved the FX5 to have a little less surface agitation, but it still does some.. and I turn my HOB off at night so I can sleep.. soo last night there wasn't thaaat much surface agitation

Edit: Oh yeah...... nitrate's aren't looking good at all........ AT ALLLL............................ fuuuck /:

Bummer man. At least it wasn't a total tank loss. Do a water change or two until nitrites read 40 ppm at most. 10-20 is better, but I'm not sure your survivors can take the stress of the water changes it would take.
