Not really an expert..... But I would be almost certain the fish died of shock... Levels not correct. I had three angel's do almost the same thing. Good luck

It shouldn't have been a problem but this doesn't explain why the discus went berserk. Did you do anything else after introducing the discus? What foods did you give them? I'd still retrieve the water tests though just to be sure and think the acclimation procedure over. Try drip acclimation on the new batch if you wish to try again.
