Could try giving him some ornaments/wood/plants to interact with? I've found my cichlids get bored in sparce tanks, they perk up a bit when you add things or move stuff once in a while

So he is in a 50 Gallon? Yes he has space to move but not to "freely swim". See how his tail fin is arched down? That can be from two things, first is genetics, but second is cramped living space which this guy is living in. If you could put him in a 90 or larger that would help him a lot, plus as convict360 has said since he is in a tank with no gravel, tank mates, decorations, or fake plants to mess around with he is probably just really really bored. If you can't afford to get him a bigger tank then at least give him some gravel he can move around, it won't crowd his space but it will give him something to actively do.

Maybe ill move his old decoration rocks back in. I was worried that might cramp his space but it makes sense what your saying. I thought he was bored or sick...

But getting a new tank won't be an option

I'd maybe avoid the rock, cos that would take up needless space, instead I'd suggest some free flowing plants like Java, or if he wrecks them; some anubias tied onto redmor wood. The advantage of redmor is that it is very attractive, yet is light and doesnt take up a lot of volume.

Sand is also your friend, and would help to accentuate your Flowerhorns colours :) just a suggestion though, each to their own

Have you added Gravel yet?? I want to see this guy start feeling better!! DONT ADD ROCKS or anything that is going to take up space because he is already too crammed up. You could add plants for him to play around with too. Post some updates when you can.

Do you guys think gravel is the issue or is he sick. He's been like this all year and first half of last year I though it was a issue with hex. But since he's alive I don't think I was a sickness. He doesn't eat like he used to either.

My gravel is in storage. I thought lots of people runs bare bottom tanks though
