He could of wedged himself under the wood, then tried to squirm and wiggle his way out thus tearing his fins and skinning himself.
 
Looks like he took a nice beating. Usually they are on the other end of the beating as the beater.
 
what exactly is in that tank?
 
I've found that my red devils will try to fit anywhere they can. Make sure all potential hiding spots are definetly big enough for them to get in and out of.

I had a livingstoni wedge himself between a heater and a rock once. I dunno how long he was there, but I freed him and he was pretty skinned up. Who knows how hot the heater got when he was trapped there.
Eventually he got a really bad infection that I just couldn't get rid of and ended up dying.

Kind of looks like that's what may have happened here.

I think the plecos fins being torn is a completely seperate issue. It's not uncommon for Leporinus species to nip at other fish, especially if they don't move much.
