The position of the hole relative to his own pectorial fin makes me think that he could have stubbed himself when strongly bent, perhaps escaping a predation attack or some other awkward situation. Cats are flexible enough to stub themselves when the pectoral spine is pressed against the body while the body bends a lot.

could be. when i see him shed he trashes around a lot.
 
As someone posted above, The Op's tank having 2ppm nitrite was the likely cause of death, The fish was probably injured and the bad water parameters cause the fish to suffer from the injuries. I don't mean the be the one to bring it up but.. you have 2 fish over 12" a 6" payara and 8" ornate in what appears to be 80 gallons of water, you are obviously over stocked.

i didn't think 2ppm was all that bad. i didn't even think it was bothersome till it got around 5 or 6 ppm. i accept your theory though as it is a high possibility. i just want to add when you say two fish over 12", my rope fish doesn't really count, yeah there're long but there're still small and very little girth. filtration is a fluval 405 and a fluvial G3. i do at least one 20-30 percent water change every week. i also feed once every two days(pellets) and tilapia once or twice a week always scooping out un eaten food.
 
Nitrites of any amount are not good, it tells you either your tank is uncycled.. or your filtration, inparticular biomedia are inadequete. nitrates however are generally acceptable up to 20ppm. the only number that acceptable over 0.

the pim imo may have also done the damage.. but its possible the lima got trapped and tried escapeing, possibly behind the heater? catfish can be very territorial ime pims can be nasty.

No I have the heater tuck away and it has a guard on it. He usually hangs out under a tree root and sometimes the pim accompanies him.

If you have nitrites I doubt you have nitrates yet, sounds like you tank is halfway cycled and your mix of fish is not recommend, so that's 2 negative factors working against you.

You have the bb that converts ammonia -->nitrite but not the second bb that converts nitrite-->nitrate.


I'd do 25% every few days till you read n03, I commend you for seeking help, we will get this worked out and your tank will be healthy in no time.

Thank you everyone for your great help. I really don't want to lose these guys. I think it might be bad water because my payara and BP are showing visible signs of disease or sickness. Payara has something wrong with his his an the BP hasn't eaten in weeks, has open sores on her head and gill curl.
