You never have to really. Unless they are corroding or something. Once in awhile you rinse them in tank water to release gunky buildup

Yes, put the bio media in some tank water.

Depends on how heavy the tank is stocked. I do bi-weekly for a heavily stocked 55.

No, but it sounds like you should rinse it very well.

Just rinse thoroughly.
5. And also when i take out the the medias, theres a lot of waste/water in the bottom of the filter, should i pour this out? or its beneficial to keep the dirty water/waste?
Its really a mix of detritus and food. I like to shut the filter off before removing the media once a month. So all of that doesn't go in the tank. I would imagine it would attribute to nitrates.
