The white strip from the gills makes me think it's a chemical burn of some sort. This may or may not recover (depends on how deep the burns) but the fish will have trouble breathing for a while so increase the turbulence on the surface of the tank.

You said you use the test strips that tell you your water is fine. Can you give exact readings? and how old are the test strips? older test strips imo are almost useless.. and sometimes you can get false readings from them.
I agree it is likely a chem burn/shock. the best thing you can do is keep your water as healthy as possible, keep an eye out for secondary infections.

well here's the link... im gna lookinto the reading right now.. the test strips are no more than 5 months old.. and i make sure the cap is extra sealed everytime im finished... ill double check readings. soon
