Not a sword, it is a Java fern and has a small plantlet on one of the leaves. Remove that wool or whatever the rhizome was planted in and don't plant the rhizome in anything.

Medium light. I have mine all throughout my tank and they don’t seem to care.

It’s directly under a light that I have one maybe 10 hours a day and off for about 14 hours

That is okay, but I would tie it to the driftwood.

It also had a root growing separate from the original plant that just got randomly buried when I was planting this fern. Is this planted right? I’m guessing the plantlet is another fern? Is this good or bad

The roots are best outside of the gravel. I do grow small ones like that though. The roots are supposed to grab onto rock or driftwood.
They reproduce by having smaller ones buds off.
