I use a couple pieces of driftwood and place a large flat rock on top. Not the best pic to show the cave but you get the idea.

Any rocks, wood, PVC, or the flower pot idea will work. If going w/rocks, try to avoid any w/sharp edges or surfaces. Lava and Lace rock are two that are pretty rough.
 
I take large flower or tree pots and break them (like in half). good for a pleco since the have a large turning radius.
 
Go to Home Depot and get some drain tile. It is made of clay and always give them a front and a back entrance. You could easily find a peice large enough for your pleco. Pvc alos works great. I like the dark grey stuff.

I have yet to have them break exactly as I want them but I usually bring the pot down hard on it's bottom untill it breaks which usually works out in spitting it. I'm sure there are more percise ways to do this but I have small fish too so the smaller pieces that break off I use for them or I use them as pieces to hold up larger pieces to make caves.

I had a thought to make cuts in the clay on the outside and inside outlining how I wanted them to break, but have not tried this method.
