Question #1 here is whether it's HITH....again Dave, take a closer look to see if there's any worm-link thingy or anything else growing in the holes...
 
I'm no expert on these types of fish, but i have seen this happen before by what i could only conclude, by the process of elimination, that it was the water being too hard.

Your typical parameters, include Nitrates, won't be the problem here.
Hi Japes... pH is 7.5 and TDS are high. Typical Chicago water that I hate. When I move (and we made an offer on the place today, so fingers crossed) I'm rigging up an auto-water change with RO to lower that all a bit. Gradually, of course.

I know the water hardness is not ideal, and as such my orangeheads have never been as brilliant as those in softer water, but nothing about it has changed in the 4 years I've been keeping this type of fish. And I can count on one hand the number of eartheaters I've lost in that time (knock wood).

It definitely looks like something is in there. Like it's a hole in the head, but something fills the hole. Either my eyesight is failing me or I'm just plain stupid though, because I can't tell what it is or describe it. It is now redder around the edges of the first hole than when I first posted this (I think), and the 2nd hole seems like it has a flat and white filling with a black dot in the center... looks like a target. The entire area around it seems kind of flattened/sunken, which is like typical HitH.

This fish actually had a cut in its lip 2 weeks ago that I just remembered. It healed completely (and very quickly) though. That'd be the only stress I could think of that it may have gone through. And right now it is behaving normally, not hiding, clamping, etc.

Good call about the disease board. I actually forgot we had one here because I always skip over that section on the main page on my way to SA/CA and Classifieds...
 
I haven't done anything, but it kind of looks like it's getting better. His head is still messed up but it seems to have healed itself a bit, the spots are just solid white now like new flesh is growing. I think I'm still going to treat with metronidazole... I just need to get some.
 
hey dont mean to hijack thread but im having the same problem but it's my little orange head. there is about 5 or 6 little holes on her face. boyfriend tested the water and everything is in check but my water is extremely hard. does anyone know if hardness or higher temps (over 84) can cause hith? also with treating the tank, would any of it effect rays? or should i take them out just in case?
