overdose and high bio load I think it is possible that the amount of dissolved oxygen was reduced to a level that it inhibited the the aerobic nitrifying bacteria.
Hello; Yes this is what I was driving at in my convoluted way. The SAFE being a reducer might be reducing the O2 as RD mentioned for a while. Maybe not enough to kill off the bb but enough to shut them down a bit.

stopped keeping the species that had problems with my tap water.
Hello; Yes to this. Maybe not a solution if your dream fish don't do well but the fish that thrive don't die as much.

Bio stress guard aka BSG 4th day with slightly more increased feeding of frogs

Followed by safe 4th day . Feeding on 1st and 3rd day. No frogs

Good thing is I don't see those white blotches on the parrot fish and fire eel... So likely it could be previous overdose of safe.. will continue to monitor.

Will also be getting better quality bio media

hello; This just occurred to me. Amphibians can have toxins in or on them. Might be a wild idea, but have you tried not feeding any frogs for a time??
Actually I did stop feeding frogs awhile while still on Safe previously. About 2-3 weeks

After 8 days. No feeding for past 2 days though.

Discovered abit of slime coat issues with fire eel. Parrots was ok

Looks like overdose of safe was the issue
