Sick puffer - Can’t seem to figure out what’s up

I never thought I’d say this, but I hope this is an internal parasite issue.
Just did some reading on myco and euthanasia of all fish in that tank as the most effective option doesn’t sound like a fun road…
 
So sorry for the loss. I hope this doesn't dissuade you from trying Amazon puffers again.

Thanks for the kind words. Still have one left - definitely going to replenish the school once this is all figured out. Even in the worst case scenario where all fish are lost I honestly can’t think of another option for the tank that I like.
 
Another thing to note, the white Molly’s gills have gotten very red - I don’t have a current picture but it looks like in the first post but even worse.
Been a busy few weeks - I’ll test water either tonight or tomorrow.
 
GL. If you haven't considered these already, some other options for if literally nothing works are smaller brackish or marine puffers (like green spotted or sharpnose).
I'm not a puffer expert but I often see these suggested for tanks in the size range of yours.
 
I am very sorry about your loss. Before reading about his passing, I was going to question the rationale of taking him out to a qt tank. Now, I think it is worth considering that what it had is also in the tank so it needs to be ‘cleaned up’ somehow. Again sorry for your loss.

Brackish always has been a consideration…
Everyone else is still kicking and I put a lot into plants, but one of my original thoughts was a fig 8. Green spot are the devil lol

I get the feeling what it has is in the dt too. By the time I put it in qt I wasn’t sure of it’s chances, but if it died in the main tank the loaches would find it before I would… not to mention it would only facilitate spread.
I lost a molly previously to internal parasites, it also died in qt (moved for the same reasons), I dosed levamisole to the tank, but very likely I didn’t clean the substrate (to get worms and eggs out) as well as I could’ve.
 
Test results:
Ammonia: 0.1 (this tests toxic and non toxic correct?)
Nitrate: <5
