Prazi mate urgently. It won't do any harm and can only help. Raise temp to 83 and lights out. That indent on the forehead needs sorting ASAP.
Lots of people Prazi just as a precaution, in this case I would say do it anyway as it can only help and can't make things worse.
Do you run UV?

I had my ray do this but not as bad,he was getting bullied by my two peacock bass and they were biting at his tail(he lost part of it)

As soon as I removed my bass he started acting and eating normal again

I'd just make sure no other fish are picking on your ray

My stingrays are with my 7pcs of 6-9inch kelberi and orino pbs, I just throw in a lot of food at one time, usually feeding the peacocks till there full then distracting them with it in one corner of the tank and then feeding the stingrays in the other corner.

You could try my method: Get a large laundry basket and suspend it within the water column(or set it gently on the floor), coax the ray into it to fully isolate it. From there, power through with feeding, keep the lights over that area dimmer and hit 'er with the old razzle dazzle salt and heat. It takes a while for them to come back around, but if you raised that puppy to that size on your own, I'm confident you'll settle this matter. ?
