Tank crashed. Don't know why

I've got a 150 gal. planted tank i wanted to use as a refusium sump for my 560. It's been running for over a year and a half.
It's full with bristlenose fry and a few other plecos. I got up Sunday morning to find over 120 fry dead, maybe more.
Last thing i did was clean out a filter, added some used bio- mac, and added about 5 gals of RO
I also added a little plant food.

What kind of toxin could it be?
There's also some airated bio-balls on the other side of sump. But they've been there for about a year.

560 isn't running yet, but all the plumbing is done. That was finished over 2 weeks ago and i don't think that has anything to do with my problem

Any ideas?

I kinda thought that. But i had my hands in there for about 3 hrs last night. I should have noticed something.
The thing is, not all of the fish died.
5 of my larger plecos r still looking ok
And there are still a couple small fry still alive. But i know after w.c last night( 80%). I still lost some
Very sad day

What are the water parameters? Smaller fish are more likely to die if ammonia spikes. What water did you use to clean your filter last night?
 
Well i didn't check water. Just did a big water change as quick as possible
I used tap water just to rinse filter pad, like i always do
Could plant supplements go bad?

Man, that suck...sorry for your loss. Hope you can at least figure out what happened, just to know and so you can possibly prevent it in the future.

Fact of the matter is, i may never find out what happened.
Tonight after work, I'll check water parameters before i do another water change. Got to see what it is

Are you using a dechlorinator?
Milwaukee uses chloramine in the tap water at a concentration of @ 1.25ppm, so depending on how far you are from the treatment facility (7th and Howard on the south side, Lincoln Memorial Dr on the north, this is enough to compromise fry but probably not adults.
If you are east of 20th street, you would see at least 1.00ppm, if you are as far west as Hwy 100 probably 0.50ppm or even less.
Also at this time of year the tap water in the our pipes is under pressure, very cold (upper 30sF), and because of these factors, supersaturated with oxygen, if you ran new water straight into the tank there is a possibility of creating gas bubble embolism in fry.
I always try to aim the new water first to a sump, or cascade against something to rapidly disperse the oxygen super saturation.
 
That all makes sense, but the problem I'm having started before the water change. I only did water change because of the loss of fish.
But the worst thing is tonight i lost my 5 year old Gold Nugget.
And there's also about 4 fry left, different sizes, out of 200 left still alive

It's got to be something really toxic. All these fish died within 30 hours ( ish)

I checked my water parameters. Perfect! Has never been that great
pH 7.2. Nitrite 0. Nitrate. 5 ppm

Guy at my fish store thinks it could have been a Co2 burp. Something that happens when Co2 bottle runs low.
But i think my pH would have been much lower. And the bubble counter's at 2 bubbles per 3 secs

I don't understand. Need to find out what happened
