Tumor in mouth?

That's a little harsh, but I do see some cause for concern as well. The only real way to keep fish in healthy condition is space, clean water and a healthy diet with no aggression. Anything less will bring stress and with stress a lot of negative things can happen.

Stir sand once per month or methane can build up and harm the fish.

I'm assuming since you have a 5" sand bed in freshwater that looks like its never been stirred, and he's with a rtc and a large bichir in a tank that's obviously not big enough(he's longer then the width of your tank) that he is in poor health due to water conditions, I'd love to hear your parameters. I really wish the average lfs wouldn't sell these giants to people without adequate habitats. Poor thing.

What size tank is he in, 55g? What's his tank mates? Water change schedule?

They're actually in a 150g. For some reason that side of the tank is their hang out area and they all go to the side during the day. When the lights are off, they start utilizing the entire tank. I have 2 large hang on back filters and a canister filter with a uv sterilizer. Ammonia and nitrite are 0 and nitrates are detectable, but low. I do once a week water changes of 25-50% and temp is between 76-78F. His tank mates are what you mentioned plus a large clown knife.
I'm not new to the hobby at all. I new every single one of my fish would become monsters. Once they outgrow the tank, they're going into my friend's heated pond. I'm not here to be ridiculed. I just want to know what's wrong with my fish so that I can do something about it.

I didn't know you had to stir the sand. I would do it occasionally, but no where close to once a month, maybe once every few months. Thanks for the input

And unless your doing saltwater, you shouldn't have more then an inch ideally

I actually have a lot more experience with saltwater so that's why I thought a deep sand bed might work with freshwater as well. I just did a 50% water change and stirred up the sand bed really well. Should I just wait it out or is there any meds I can give?

Pretty far off, if your tank is 18" he's at least 20"....

How are you even making these guesses when I haven't even posted a side view picture of him? He's not even close to 20", not even 12".

Here's a pic of both my rtc and tns.

The red tail is like an inch shy of being as long as this tank is deep. He's just turned in the pic, but my tsn is straight and he barely passes the halfway point. the water is still murky from the stirring of the sand.
