increase your water changes

so how often should I do water change? Twice a week?

Hello; How often and how much water you change will depend on the water conditions. The goal is 0 ammonia, 0 nitrites and under 20 pmm of nitrate. You will need a good test kit to know these things.

A word of caution. If your water is indeed high in nitrates and you have been doing less that 30% WC each week, then do not jump to 60%, 70% or more WC all at once. Some problems are possible with too big a WC if you have not been doing them.

Without a test kit my I suggest 20% to 30% every other day for a couple of days. Then 30% to 40% every couple of days. Then 40% to 50%. A WAG is that you will need 50% or more WC about twice a week. The test kit will give you a better indicator, being when nitrates approach 40ppm that is too high.

One more question after the water turns clear how much % of water should I change per WC?
 
Does anybody know why my tank water looks like this? Is like a yellow color n kinda cloudy..

It's a heterotrophic bacterial bloom. The heterotrophic bacteria live off organic carbon in the water, not ammonia or nitrite like the nitrifying bacteria. The bacteria in the water column (heterotrophic bacteria) normally go away when they take up residence in the biofilter's ceramics, sponges and the substrate along side the nitrifying bacteria. However, there needs to be enough surface area and enough water flow for the autotrophs and heterotrophs to take residence in your biofilter. What filters are you running?

It depends on your water and your bioload. If your water is very soft and has very low alkalinity (KH) and you have a large bioload (ammonia) then you may need to change 75% weekly just to keep pH from crashing into the 5s. Typically you perform water changes as needed to keep nitrate below 25 ppm.

Like Tarheel says, depends on exact water conditions and how much it actually takes to keep everything stable. But 50%/week is usually a good starting point.
I usually do water changes every Friday but guess is to long because it gets like yellow color and cloudy look i did a water change today and by Monday or Tuesday is gonna be bad again..
