What am I dealing with

Not hith....looks like a scrape or chunk taken out from hitting decor.

Would get some hydrogen perxodie and dab the area with a qtip and place back into the tank. Can do this once to twice a day for a week. Check the progression.

Keep updating if it gets worse. Hope it gets better.

Not much. Look like a bad scrape. Try the hydrogen peroxide and keep the water clean. Should resolve.

Keep updating after a week or so.

Maybe it could be some sort of external parasite that doesn't cause much harm? Maybe your fish tries to scrape it off and it re-irritates it, kind of causing a reoccurring cycle?
 
A year?! If it was an infection then it would have progressed atleast or killed the fish. Maybe scarring.
