Why did my puffer die (pics)

If you have bubbles all over the rock/walls, you have super saturated your tank. It leads to air embolisms in the fish. Not a disease at all, just a physical reaction they can't live through.
I would guess at some point your filter return or power head started to pull air in from the surface, and spun it into micro bubbles via a tiny venturi. It happens more than you'd think.
 
cold water? It retains O2 that expands when meeting warm water causing a bulbing effect.

Kind of like when divers get "the bends"

I'd figure out where the bubbles came from. Then go from there.
Was the tank low at some point? Filter return ever snag air? How about the power head?

lol I havent been posting much I got sick of answering questions that anyone starting aquariums should know (like diatoms for example) lol. So I've been chilling in the rock/coral, and ID/disease section mostly haha.
