Your worst mistake?

Trusted a 10 year old thermometer. Got a second opinion thermomter; water temp: 86 degrees. Lost 7 swordtails.
 
Biggest Mistake I made was letting young one look at tank unsupervised came back in a few moment later they had pulled return line off canister filter drained about 40 gallons on floor to make things worse when I came in panic insued cause first thing I was worried about was them getting shocked my dumb back side didn't have shoes on stepped in electrified water and yeh you know the rest Needless to say after mess was all cleaned up
wife had a pretty good laugh about it all LESSON LEARNED !!!!!
 
^ lol sounds like that should have been in a sitcom, I got shocked by some hot water at work one time, except it was industrial current and I had grabbed a metal tin that fell into the water.... derp

While siphoning the tank i didnt check up on the sink to make sure that the water was draining properly and ended up having a huge flood in a apartment. Thankfully there was no damage done.
 
I remember ages ago when i was little, my dad was cleaning out the 6ft koi tank(rip) we use to have and some how he was cleaning it with liquid (dont ask) and there was foam still at the top after various wash which i told him i still believe there liquid in the water, he did not believe me, i ended up shutting up, next day came downstairs early in the morning to over 30 dead koi, broke my heart and there was 3 alive which ended up also passing away :( the day i will never forget, even few of my first ever fish i got when i got into the hobby passed away, was more of my dad mistake then mine.
