Dent in the forehead of my ray--- but not from being skinny?????

Ok, so i have had this guy for 6 months, he is a captive bred. He is about 8-10" range.
I noticed last night while feeding smelt he did not seem interested in food.... I thought "boy that's strange he eats like a pig normally" I started examining him and noticed this dent in his forehead. I started thinking he ate the day before yesterday, how could he have this?? This morning i woke up and threw in some pellets and he jumped on them and ate. But seems a little sluggish compared to normal, and dent is still there.
Tank-mates are 2-p14, 1 leo/hen, the huge fire eel, 5 Silver-dollars, and 2 geos. which all seem to be fine.

The thing that has me the most worried is i lost a hystrix i have had for 3 years earlier in the year, and she also had the dented forehead, but she did not eat for a couple weeks after being accidently bitten by my Tigrinus so i assumed that is why.

I starting to think something has and may be going on with my water, i have well water, and i have kept rays for 6 years and this year was the first time to loose any rays, and i have lost 3 rays in 2012.
Maybe DB is not the only one with BAD WATER?

Any opinions? Any ideas?

no, i do not.
I know alot of people and that including myself thought that Uv's were just a luxury Item For hobbiest ..For myself i believe Now that i could not have been more wrong especially as Rays are concerned . What i have watched learned Rays are very tough physically they can have a wound that would certainly kill other fish yet they go on heal as if nothing has happened ....It is my belief Bacteria's virus and parasites can and will attack the immune system of these unique creatures because of there design and were they spend their time Just any Uv won't cut it also it must be matched to your tank volume and flow the correct dosage is Key or it will just be a water clarifier !!!! you must whatever Brand of Uv you use match it to your system or it will be worthless just my opinion......
 
From my understanding, you treat all your rays profilactily for parasites. So I wouldnt believe it is praasitic. Besides you will usually see signs through fecies and weight loss. If bacterial then it came from somewhere in your system, newly introduced tank mate or a wound.
I would lean towards something stressing it. Their is one importer whom I have conversed with on multiple ocassions who believes the dented forehead while on a well fed ray is totally stressed related.
Cant say yeah or nay on a UV as I have never found a reason to use one. The one ray I lost, my flower, was totally stress related.

Well he is a black ray, I just don't see anything stressing him.. If anything he should be the one dishing it out.
About 2-3 months ago he took a stinger to his eye and it looked pretty bad. It bled for a week or so, I thought he would loose his eye. It has almost completely healed since and is just a tiny bit cloudy now. But even during this he acted like nothing ever happened.
I'm not big on treating without a reason too, so not all my rays have been treated.

Looks a lot like what I went through with my female marble I lost. It also looks like his disk is slightly swollen or raised. I treated with prazi which seemed to help and she was normal for a good three months before it struck again. No other rays or fish where affected in my system
