Straight up open sore, it was caused by rotting dead pup in the filter. When I pulled the dead female out, the underside looks similar to the ray in the first page....ammonia was through the roof, it was a small 75 gallon tank (qt) with overfiltered filtration that apparently wasn't enough to handle a rotting dead pup. I am still 100% certain this one was ammonia poisoning.
I too have had ray with bloody eye, clear itself in less than a week..
