You need to make sure all 4 corners of the plywood are level with each other. You should also paint the plywood with exterior paint to keep it from warping if/when it gets wet, because you neglected to drive to philly to get the proper marine grade pltwood.
 
Everyone's been saying check your stand, check your plywood.

Have you thought of taking your tank off the stand, turn your tank upside down on a very flat and even surface and, with a long spirit level check the integrity of the tank from corner to corner on the diagonal. That will give you an answer as to whether your tank is warped or not.

You'd have to be very very unlucky for your stand, ply and tank all to have problems, I suspect there's an issue with only one of the three. It's just a case of narrowing it down and then you know where to focus.

If it's truly the tank, then I think shimming is all you can do, whether that's under the stand or between plywood and stand.

If shimming under the stand doesn't close those gaps, then gently shim (i.e. don't bang them in with a hammer) under the plywood to close those gaps and that's probably the best you can do with a warped tank.
