I did water change, and didn't add Prime right away when I added tap water. I got caught up with other stuff for about 10-15 minutes, and by the time I came to add Prime, the damage was insane! At least half of my mahseer population was genocided as a result!

Ouch, make it a priority before a water change to grab the bottle, step 1 prime in hand.

The botttle was there. I was moving some fish from other tank into bigger tank, and got distracted for a bit

What percentage do you change?
The same percentage that I've been doing twice a week for the past years.
 
Oh s$#@t those weren't smelts? I thought they look like mahseers. Never seen them out of water like that. Sorry for the lost.
 
I never use prime I do water changes everyday ... prime is pointless

That's why I was asking what percentage. I have been putting straight tap water into one of my tanks for years with no conditioner at all, have never had a single problem. I think I must do a smaller percentage than OP.

I normally change 40% per week and sometimes don't add conditioner. Fish are ok, but always made me nervous. Recently invested In a 2 stage filter that remove heavy metals and chlorine, chloride etc. You can buy similar units to purify water for RV's. Search RV or camper trailer water filter and you'll see what I mean. Works a treat and no more water conditioner. Sorry for your loss btw.
