To my knowledge epsom doesn’t do much besides help to calm stress.
If metroplex + focus is available, that can work.

Epsom salt helps against spironuclues vortens, the intestinal protozoan flagellate parasite that causes hexamita, showing symptoms like white stringy poo or hole in the head or lateral line erosion, or any combination. Using epsom salt and metronidazole, is generally the 1, 2 punch to knock out spironucleus vortens. It's why NLS hex shield as invented (epsom salt + metro).

Here's the sticky on S. vortens and epsom salt

That sounds like it would be caused by some internal parasite. Continuing to get water quality in line would help, but meds are generally needed to rid a fish of them. Levamisole is commonly used for treating parasites.

Oh bro, I thought the medicine you said was specific to the fishes. It’s available in 150mg/tablet Or 50mg/tablet for us. Mines a 150 Gallon tank.

If the tablet is levamisole HCL and 100%, you could crush the pills and follow the dosing instructions from here:

basically, 2.36 mg/L of levamisole HCL = 2ppm levamisole dosage that you need.

My Oscars get 25% - 30% two to three time a per week. Top water quality is paramount. Look after the parameters and you will have zero issues.

A 150 gal aquarium with a couple oscars, is akin to a city yard with 2 big dogs.
Your filter (like the grass and soil in the yard) has the ability to "partially" detoxify some of the urine.
That filter can also catch a little of the poop and detoxify a tiny bit of it.
But if you don't regularly clean the mechanical filter media, its basically like sweeping dog feces under a rug.
So what alternative do we aquarists have.
In the yard, we regularly shovel up the dog krapp, or walk them, so they poop in someone else's area,
if we don't well......
In an aquarium the equivalent to shoveling is a water change, or walking the fish in someone else's clean tank.......or fish (dog) gets sick..
With only 1 water change per month (or cleaning of the yard) your neighbors might complain, you may get a citation from th city, etc etc,
If your a cat person the tank is the equivalent of a cat littler box, so not cleaning a litter box, well.....
And it doesn't matter what kind , or how many meds you ply the fish with.
Unless you start a regular routine of keeping water clean, all the meds in the world is just masterbation.
