yea i just got a marineland magnum 350 and its got the carbon filter in i.
this week i fed him a carrot and a potato other than that he sticks to meal worms, and hikari cichlid gold
 
Carbon would be my guess with the pits in the head, and the white on his/her fins looks like cyst.

If nitrates are 40ppm, I'd start doing more water changes, and cleaning mechanical filter media. Oscars are very sensitive to nitrate, and 40ppm is considered a high stress concentration, which is a major precursor to hole in the head.
And any ammonia at all, means water changes, and filters are not keeping up with the waste output of the Oscar.
 
Are you running carbon in your tank and are you mixing up the diet of the fish?

The carbon thing is a myth. Only stress on fish causes HITH. This means tank mates or water conditions like high nitrates.
OP- I would do more water changes and start using aquarium salt for treatment. Oscars can recover and heal quickly.

Idk if that's HITH. Every oscar I've ever seen seems to have that same exact pattern on them. Including mine who does not have hith. However your ammonia should ALWAYS be zero. Handle that first ASAP.

Use an ammonia binding product if you dont have time for the water changes, i use an ammonia binding chemical filtration pad and Ammo-Lock (API) if my water tests for ammonia at all.
