This is my froggy splitting. (Please ignore dirty glass) Also they will get little "buds" on the side.

I cant see anything in the picture.
I'll agree with that. The skeleton is cracked. Did it fall? If it was splitting the head would get bigger and slowly split into two separate heads. Like this. There are 4 heads on mine in the picture.

I wouldn't necessarily call it a 'fall', but when i couldnt get it out of the plastic bag for acclimation I had to 'roll it over' 360 degrees, twice. It was upright then on its side then completely upside down then on its side then upright (twice). This lasted 5 seconds total. It appears to have a deeper crack now and 1 side the seams seem to be coming apart and the base is splitting.
I have bad news btw... (to be continued)
I found my yellow tail damsel dead about 30 minutes ago. I also found a snail that fell over on his back and died yesterday (astrea snail).His eyes (damsel) had been eaten and he appeared discolored and half his scales were missing. He looked beat up. I took him out and immediately and thoroughly tested the water. I shook the bottles nonstop for 5 minutes, then shook the vial, and waited 5 minutes for it to mature. Lots of green algae in the tank. Purple algae is also starting to appear very slowly.
The parameters seem fine:
Ammonia 0 ppm
Nitrates 5ppm-10ppm

Happens, just make sure there are no signs of disease.

Look at the head in the right, notice the "crack" down the middle of the head. That is how they normally split. http://www.monsterfishkeepers.com/forums/attachment.php?attachmentid=869130

I figured out what the goby is. A Yellow Clown Goby.

Do you have more Damsels? Could he have been killed by another fish?

Clowns get territorial. It could have possibly been the clown that killed him
