Do you know what caused its death

Yes, I had a faulty wave maker that cause the fuse to the tank setup to go flip off and so when I pushed it back up, the wave maker blew up inside the tank (literally). With smoke fuming and heavy toxins, I quickly try to do an emergency WC but at that point the gatf was flipping out like crazy and going full speed slamming into each side of glass. Ultimately, she knocked herself out and damaged the swimming bladder and was not able to balance herself out anymore. I tried to hold her steady and keep oxygen stones blowing right in her face but after a few hours, the damage was irreparable. Quite a shame, she got use to the small space but the underwater explosion was just too crazy...
