Id stick it in a bigger tank. I kept my 6" trio in a 6ft tank. I wouldnt have them in anything less then a 50. Is the tank in a spot where lots of people walk by? also have u added salt?
 
Bigger tank. Another thing is to cover the whole tank with a black background. Worked with my armatus.
 
Oops double post sorry the tank is black in the back not on the sides and yes I add a little salt once a week but do water changes 3 times a week and the fish is in my fish room no foot traffic but me I will try to locate a bigger tank but don't have one at this time will also cover both ends thanks for the info

Give him some decor to hide behind, payaras can be very skittish in the beginning. You need something in there to break his line of sight to make him feel more secure. Also blacking out the back and sides like lix suggested is a good idea as well. Payaras like to attack their own reflection in the glass, just add some salt and bump the temp to 80+ and he should heal up in a few days.

Edit: I also highly suggest moving this fish to a 50+gal asap.
