Probably not sinus. The pulp, or tooth nerve is a pretty fragile thing. Any trauma, including deep caries (tooth decay) can cause the pulp to deteriorate and die. If your pain is because the fractured tooth is moving when you bite down, you are not going to get relief until the broken piece is removed. Dental procedures require cooperation of the patient. The patient has to lie still and open wide for a dentist to work. If you can't do both, you may have to find a place where you can have dental work done under general anesthesia.

How unfortunate for you. Without routine dental care you run the risk of losing all your teeth. Dental problems NEVER resolve on their own. Little holes just become bigger deeper holes. When dental caries gets really bad, the tooth is essentially rotten with a thin eggshell layer of enamel on the outside. The tooth can easily break. Very sad because a little filling a few years earlier could have prevented loss of the tooth. It sounds like you may have a whole mouth of neglected teeth. Hopefully you can find a hospital setting to get the one tooth dealt with. Hospital dentistry is usually for impaired children and adults, but you fit that category right now. Perhaps in the future, you can get psychological intervention to enable you to get dental care in a more traditional setting.
 
I had 3 teeth pulled out a couple of years ago

could only get over the counter stuff for the pain. was hell for first 3-4 days
 
Yuckmouth-classic...I remember years ago when I last had to have a tooth pulled I did not have any dental insurance at the time and went to this place as an emergency walk in.I had to pay seventy five dollars out of pocket to have it pulled but the pain from the toothache was excruiating and it was money well spent.

I had a procedure done a year ago, they gave me a pill called lorazapan I believe. You will need a driver to get you home but it works great. I'll describe it like this, you are so relaxed that they could cut of a toe with a butter knife and you wouldn't mind. Ask the dentist about it.
