Selling my BEASTY 8" pacu to my friend and it killed his 14" arow,9"pbass,his rare puffer,and something else lol
 
lost my jardini aro because it jumped out of my tank to catch a gecko.i didn't have a cover for the tank.when i came home i saw the stiff jardini on the floor.since then i have never kept another aro. :(
 
#1 Not having fish euthanized when I moved away to go to college. Too many flaky people who were going to pick them up and never did. Too much suffering for the fish left behind who waited too long for homes.

#2 Related to #1, buying a blue eye pleco who nobody wanted after I moved away and died as an indirect result of being boarded at my parent's house.
 
I don't know if this is my biggest mistake, but it's the most recent. From now on I'll always make sure the lid is on tight when I'm setting a brand new bottle of prime under my stand when there are curious kittens playing nearby.
 
my biggest mistake was going into the fish room for feeding and opening the black arowana tank and the phone rang so i went to answer it and forgot to shut the door he jumped out and killed himself :( he was my favorite fish 2 and a half feet long paid 150$ for him when he was a baby
 
Mine was keeping my tank temp at around 88* and PH too low for a S. American prawn. While it shed quite often and seemed to thrive it finally met its demise on the last shed. My guess was that I ran it at too high of a temp and low of a PH. Shed way too often and couldn't properly harden its shells in between. Poor thing died really slowly and kept fighting off its last shed. Now I keep PH higher and stable along with a lower tank temp 83*.

Might not seem like a disaster but I felt bad for the little guy.
