whats your biggest mistake

Biggest mistake was when I was 12 or 13 and got my first cichlids, a texas and a jd, and put them in an un-cycled tank. The "tank" was a 3 gallon critter keeper without a filter or a heater. They lasted 3 weeks.
 
A couple of years ago my professor ordered some planaria and other inverts for our neurobio class. I worked in the marine lab, so when all of the organisms he ordered arrived, I had to put them in their respective tanks.

We had covered a lot of different types of flatworms, both marine and freshwater....and I didn't realize the planaria were freshwater...

...After 5 minutes I ended up with what looked like tiny brown potato chips. I quickly went online and ordered another batch without telling my professor they had arrived. I also put the little shriveled ones in freshwater and some survived!
 
My biggest mistake was last night... I work 3rd shift and about 2hrs before I went in I had to take an oscar that I rescued out of my 180, he was being too aggressive and I had planned on taking him to a LFS.

Well I got him out and put him in a home depot bucket... was in the process of setting up a 10g quarantine tank when my wife needed help with my 9 month old daughter. She is teething and becomes possessed. I ended up running out of the house almost late for work and forgot to put the O in the tank. Needless to say he was belly up by the morning. I need to go to church and confess... I feel horrible.

I shaved, put on some after shave lotion and decided to do a WC and scrub the inside glass on a 55 of africans I had WHILE having forgotten to wash my hands... Not sure what it was in the lotion, but it gave the whole tank velvet. It was too late by then time I figured out what it was... I only saved 1 fish out of about 20... Lesson learnt? ALWAYS WASH YOUR HANDS WITH PHOSPHATE FREE (preferably enviro friendly) SOAP BEFORE PUTTING HANDS IN TANKS!!
