It's Hex treat with Metro.
 
Metro is Metronidazole. But after seeing these pics try API Fungus Guard. You should treat the whole tank. I don't think this is hex now that I see the new pics.
 
tank size, water perams, water change schedule, number of fish in tank and size? and any thing new added to tank lately?
 
you can try pimifix and melifix. I would do a fin level water change before dosing. if that not work time to hit the heavy guns.
 
never mind, bow to aquanero, not looking to confuse you, i would as he suggests. but do a fin level water change first.
