I had to treat fish like this at the petstore I worked at that had hexamita. AKA hold in the head disease. It only pops up along the lines of lateral and don't involve pustules. I had a goldfish with it once. Despite me knowing what it was the the help of my boss (a vet) I still ended up loosing the fish. If you got a good stain of it I could help you out with morphology.

Could still easily be hex with a secondary fungal or bacterial infection setting in
 
Hex does not only occur along the lateral line, that's just not true. Early stages of hex will present as white tufts protruding from the head face and around the gill area.

You could also try in conjunction with treating the tank NLS Hex-Shield it works.
 
I have used the API general cure three different times and salt each time and it clears up for a couple of weeks but it comes right back. Is there something else I can try?

I am using nls hex shield along with salt and when it reoccurs i use general cure and it seems to clear up but keeps coming back. Is there anything i can do to keep it from coming back?
 
what are water perams running, and what are you using for a test kit.
 
Keep in mind multiple treatments of metro will lead to organ toxicity issues, I wouldn't recommend more than two cycles....Metro being a antibiotic will treat a whole range of issues from internal parasites to bacterial infections.
