When I was a teenager I had two Oscars. The one would come up and tap the tank cover every morning for feeding time. While I was away for the weekend my parents shut the door. Well, the one Oscar got really hungry and hit the cover so hard he jumped out. Later that morning my Dad found him on the floor. I was pretty upset. From that day forward I've always had a hood for my tanks.
I also had the thermostat on a heater die and practically boil the water. It was up to 92F. I did a small water change to help bring it down. I just opened the hood and let it come down naturally. I was afraid to do a big WC in fear of shocking the fish.
I lost 22 adult L46 Zebra plecos that were trying to breed trapping females in caves, and had breed once before.
I was out of state training with the Army and the wife came to visit and had a so called "friend" take care of all the fish and had her come over to help feed with her for a week. We came home 2 weeks later to find a whole can of food in the tank with all the plecos turned to mush and alot of other fish dead in other tanks. Needless to say the "friend" never answered her phone again, and moved to a different state before we got home. LUCKY FOR HER!!!!!!!!!!

I heard of an aquaculture pond that lost all its ten thousand fish over night because a transfuser box blew and sent out electric waves that melted the water pump and stopped all oxygen flow. The fish died of suffocation.
When I first started keeping fish, I was about 8 years old and had a 5 gallon tank with goldfish. This was before I learned about cycling the tank. I was wondering why my tank kept getting cloudy and fishy smelling a few days after doing a complete water change.
So I kept changing the water every few days when it became cloudy. I also couldn't figure out why the goldfish kept dying. Boy did I buy and lose a lot of goldfish. The Doctor.

The Doctor.not my tank, but at a party as a kid I remember a bunch of folks saying, hey lets get the fish drunk...........they proceeded to throw a couple of beers in this kids dads 180 gallon reef..............everything was dead hours later.
not a reef tank but still ... And it wasnt beer but moonshine. The party ended pretty quick and the jokers got one helluva bill to pay
