Worst tank accident?

Mine was when my RO unit had a water line pop off and just started spraying water with no resistance from the filter. I wasn't home and by the time I got home it had the bathroom flooded so bad that it flowed into my bedroom and into the living room. So I had to turn off the water and hook up the python to start sucking off the water that was everywhere then I had to use my towels to clean up the mess. I now always turn off the water shut off to the filter and I made some plumbing changes so that it would be unlikely to happen again knock on wood.
 
2 years ago, there was a huge winter storm that made the power go out.
no power + tropical fish = tragedy.
to make matters worse, i emptied the water after the fishes died but forgot to unplug the heater or the filter. the heater broke
 
i had the same problem that stingray12 had... i bought a bunch of feeders to feed to my tank and i lost about $300 worth of fish! talk about angry! i knew i should have done it too but i said "naw...should be ok!"
 
I just had my air pump turn into a water pump two days ago.... i have 2 outputs ran into a T then into one line. One of the pieces from the T came off of the air pump and turned the air line into a syphon. About 15-20 gallons of water all over my carpet.....
 
I always forget that my 20 gallon takes only a few mins till fill up during water changes with my gravel vac and it ends up over flowing on the floors done this three times.
 
i had one couple days ago...turned everything off on my 180 accept the 2 150watt heaters since they are plugged behind the tank not realizing they would get hot after i drain the sump to stuff some sponge in it..after i drain and stuff then refill the sump i hear a "pop" noise and a sizzle.. i figured my dog was some were around trying to lick ass.so the sumps full now and i go to adjust the pump inside and get shocked i wack my head on the inside of the stand and swing my arm so fast to try and get away i end up breaking off the pvc joint that goes from overflow to sump.now im covered in water and just got shocked underneath the tank. now trying to unplug heaters with a stick i had...lesson learned...listen to the sticker on the heaters..dont leave plugged in while doing a wc. and i had a 20g the cracked once while filling in my room.but didnt leak luckilly:)
 
First, came back from a month in Las Vegas to find that my friend who was feeding my fish didn't feed them more than once a week. Tank was a 20L with 8 small african cichlids in it. My friend went by the day before I got home to "Make Up" for not feeding them more while I was gone, He put a months worth of food in at once. Needless to say, fish were dying when I walked in. Tried to start an emergency water change, bumped a rock, rock fell, broke front pane. Water started to trickle out. I told my wife to grab and end, we started to carry the tank outside. We make it out the front door where my wife couldn't hold it anymore. She dropped her end and the tank exploded. I had fish flopping in my flower beds. Managed to save about 4-5 of the fish though.

My favorite was when I was deployed to Qatar in 2003. I traded in my fish from my 2 55g tanks and just turned them off with water in them. Water was gross so I told my wife to just dump some bleach in them and turn on the filters to start to clean the tank. I figured I would just start all over again. Well, we didnt have liquid bleach so she dumped in Tide w/bleach. When she started the filters everything just bubbled up all over the place and made a huge mess. Took a while to clean but it was worth it because I still laugh at it.
