I laid down a brand new 55gal tank on its side to remove a sticker from the bottom of the tank and laid it flat on a marble in the living room....cracked the whole side in to hundreds of pieces...
Ouch...
I had a 29 outside rinsing it out. I filled it up and then went to dump out the last little bit of water after I drained it. Went to tip it on its end and dropped it when the corner was only about 5" from the ground, but the one end shattered.

Another time I was carrying a 29 past a 10 gallon QT and put the corner of the 29 right through the end of the 10. At least it was only a 10, but 10 gallons is still a lot of water to clean up off the floor

I dropped a 75 from the top of a pail while I was cleaning out. I have also over filled tanks. One time I got an order of fish and I forgot that the once end of my tank drilled and plugged so I am adding bags of fish and then all of the sudden I hear running water and can't find it.

Five years when i just started the hobby I knocked over a 10 gallon with fish and everything. I was on a metal stand that had a piece of wood on the top. The wood slight slid just a little bit every day, until the tank was really on edge, then i hit it, and.....

I've also shocked myself 5-6 times, really need to get one of those GFCIs.

Dumbest thing I ever did was about 9 years ago or so, when I was moving, I set up my piranha tank at my Dad's temporarily. I left them there for a week then put some feeders in and figured they would be fine. A few days later I get a call from my Dad that both my red bellies were dead. So I went over there and found that a feeder had blocked the intake tube of the filter, basically rendering the filter useless and killing the fish. My Dad didn't keep fish at all so he didn't even notice until it was too late. A 99 cent intake tube screen would have saved them. Also, never leave you fish in the care of someone who doesn't keep fish (not my Dad's fault here, MINE!)
I set the tank up again at my new place with new fish. My roommate would rearrange the furniture every few weeks. The tank met it's end when he tried to move it without taking out enough water first (I wasn't home) He said he had about 2 more inches to even it out and lost control. 40 gallons of water, broken glass, and flopping fish on the floor of our apartment......
If I remember correctly he saved a pleco and some other fish but the rest died.

havent made too many mistakes yet... im sure i will at somepoint though..
a while ago i turned 6 balloon rams into 1 within 48 hours.. im thinking it had something to do with the 6" gibby, 5" CAE, the handful of corys and assorted swordtails in a 30g with only a whisper 20 on it...
the 2 pbass in my 55 community tank have made short work of my ghost shrimp... so far ive still got all my otos.. for now..

bringing home a 210gallon fish tank that almost got me and my dad kicked out of the house my mom was like:WHOA::WHOA::WHOA:

my biggest mistake was the fish i bought..i thought they were cool untill i got them in my tank i got 3 tiger barbs, 2 black skirt tetras, peacock eel, african brown knife, and two oscars...the only two that i kinda like are the oscars now im stuck with these other fish i dont like they are just taking up space. lesson learned.

About 13 years ago I bought a small RBP while on vacation (They were not legal in Fl or at least I had never seen one) as well as a 10g tank and battery powered pump and a case of D batteries to last the drive, 2 days in the car. Lots of batteries and so on. Along with water splashing the whole way in the back of my Grandparents van (I was 13). So anyway long story short I get this thing home, which hadn't eaten in a week and toss it in a completely uncycled tank full of tap water. Dead in .02 seconds. Lesson learned, cycle your tank! I was really bummed at the time. Funny thing is now that I can buy RBP legally here I've still yet to get one.

About a year later I got a 29g that I thought was HUGE at the time and had a juvi Oscar in it. I was at Petland with some cash (Rare as a kid) and just HAD to have these two large JD's. I didn't know what they were but I wanted them. I was going by the Inch per gallon rule. Anyway, within 10 minutes they were fighting with each other and the Oscar. It was bad. I put them in a 2g hex tank I had and marched right back up to Petland to trade them in, mean bastards.

Then when I was like 15 my Father got a 55g to put some native Bluegills in and it busted a seam because the stand was crap, at least thats what I still think to this day. Anyway it was in the garage to get resealed and I accidently shot a hockey puck through it.

I've also walked away from a siphon in a bucket a few times before (got a Python now) and ended up with gallons on the floor. The carpet in my bedroom has a slight tint in one spot and I know why, haha. Well I guess in my living room there might be too but it's not as clean so harder to notice. It's crap carpet that stains uber easy anyway, I hate it.

Most recently I mixed my DIY C02 WAY to strong and dropped the PH from 7.2 to <5 overnight in my 75g, needless to say I lost my OLD Chocolate Cichlid and Juvi Arowana. The Plecos and CAE's of course survived fine. Always the cool fish that are weaker. Needless to say I have pulled the DIY C02 off all my tanks after that.

I may be forgetting some but those will do.
