My air stones in my tank kept getting knocked loose by my cats. When i grabbed em to burry them back under the gravel they crumbled in my hands. Being that i didnt have any more air stones i just took the tubes out since they were blowing huge loud obnoxious bubbles. I figured the fish would be ok until i could run to petsmart the next morning. that was 10:00pm.... 5:0am wake up to take the dog out and go into to check on my fish. All dead. My 18in RTC, 12in Cichla Mono, 14in TSN, 14 in Niger, and 14in RTCxTSN... the only survivors were the two ugly ass gold fish.

WC? What Water Change. When I was new to the hobby, I didn't know the concept of water change and did the most disastrous thing that noobs do, which is to put all the fish in the bucket and "clean" out everything in the tank including the filter and media. This led to a few deaths.
Arowana Diet/ Mixing Meds. My first aro was a Green Aro, I had it for over a year I think, it died when I fed it dead Super Worms. Not a good idea as they might have died from disease and became contaminated. My second aro was an RTG, it died when I fed it feeders, it got Anchor Worms from the feeders, when I medicated, I tried 3 different kinds of meds and at times they were put together one after another. My third arowana was a beastly 30" silver, it died when it jumped out of my pond.

Excitement: I heard there was a chicla on one of the LFS. I was driving my car with my wife and asked her to call the LFS. It was a red light and I am on the phone with the guy and i asked him to reserve 2 chicla and suddenly the green light turned on.. so i step on the gas without noticing that the light is already warning and I crossed the street and the police signals me to stop and gave me a ticket LOL..
The next day i went to the LFS and get the chicla without even noticing (again) that one of them is sick. The following day the sicked fish died.
Lesson learned.. never rushed things...

if fishing counts i used a 20lb swivle for sturgeon... I had one till it jumped out of the water and broke me off.
if jut fish then probably not filling my tank up all the way before i went on a trip, it evaporated below the safe point of the heater, the heater cracked, and the water went frigid luckily the all but 2 of the fish survived

Newest big mistake: Believing the husband when he "promised" he really wanted to learn how to maintain the tanks and would HELP if we got yet another one. This makes #3 he has fish in, swore he'd help with, and doesn't even know what's happening in.
Found out yesterday that "his" 55g has YET to have it's filter cleaned after 4 months with fish in it. I tested the water and the nitrates were really high (80ppm or more) and the pH is at 6.4 which is unusual. So I went to pull out one of the filter pads during a water change and it had all but completely disintegrated.
Yeah so, come to find out, he assumed *I* would take care of the filter after the fish were put in since he "doesn't know what he's doing" meanwhile every time I ask about WC's and maintenance he says "oh yeah, I just did all that on my day off"..
No more tanks or fish for hubby unless I approve

Also learned, hubby does not know how to test water despite me teaching him at least 5 times already. I keep notebooks for all my tanks and gave him one for his. He has never tested "his" tank.. Uh, yeah, methinks hubby will just continue to support MY fish hobby..

When I was young I never did a water change. I would "clean" the tank once every few months. I also under filtered everything. This included two Oscars in a 20g tank. I grew up having well water. When I first moved to a place that had municipal water I filled my 55g community tank w/o even thinking about dechlor.
