I agree aqua, want to make sure nitrate test being done properly.

ok, so rince the bottle in tank water, not tap water, then fill to level shown, shake test bottle #1 a bit, this one not need a whole of shaking and add the 10 drops to the vile, then test bottle#2 shake the pee out of it, beat on a table what ever. the stuff crystallizes and needs to be broke and dissolved before adding the ten drops to the test water. .
 
and when was last water change? and how much? and what has water change schedule been for past 5 months?
 
Look at the eyes the fish is stunted. Up your water change schedule and filter maintenance. Do a very through gravel vacuuming while changing water. This is definitely a water related issue IMO. Clean the filter media in a bucket of tank water and soak the food in some vitamins for the next few weeks if you can. These fish can sometimes be brought back and grow normally but you have to stay of top on maintenance, clean, clean water will be 99% of the difference between success and failure here.
