Just a FYI - Nutrafin Aqua Plus does NOT remove chloramine. (yes, I know what it says on the label)

What it does is break the chlorine/ammonia bond, leaving your bio-filtration to deal with the resulting ammonia spike, which can vary depending on the size of the water change, and the mg/l of chloramine used by your local water municipality.
Trying not to derail but is it a good or bad thing? I've used nutrafin for so long & never had problems.. & it's the best water conditioner I can get at my LFS..

Btw, does your fh have white spots? I couldn't see it in the vid..
It could be something else, internal parasite maybe?
My friend had a fh that has pop eye & stayed at the surface for a long time.. it died though..

Sorry buddy but it isn't a case of ich. They usually go up to the top for air when they aren't getting enough, with his scratching at the gills he most likely has some sort of parasite in there that is sucking on his gill filaments(I think they are called) which are where his deoxygenated blood get re-oxygenated. They are depriving him of precious space on his gills where they feed off his blood. In many cases it is flukes. Which is what I think you have. I know I commonly tell alot of people that it may be a case with their fish but it is VERY common in the fish trade. If you look here you will see your fish hass all the symptoms http://ezinearticles.com/?Skin-and-Gill-Flukes-in-Tropical-Fish&id=78794
Many people associate scratching with ich... That is never my first thought anymore.

Until you know what is really wrong with your fh, I don't recommend you buy more meds.. Sorry but I'm not good in diagnosing diseases..
If there's no white spots, it's very likely to be parasite..

I have never heard of it but I think it may be your only choice.
When treating flukes though you want to be sure to treat for a whole month, that way you know for sure you get it all. I'm just worried that it maybe too late.
Ask them also about tetracycline and if they have it. I have heard that it works for flukes also but I was too late with mine http://www.monsterfishkeepers.com/forums/showthread.php?t=253662
