first things first, did you travel back in time on the way to your local fish store to get the dechlor and medicine the first time?
Secondly, like RD. said stop random treatments of meds. Over my time in the hobby I have killed more fish using meds than letting whatever they had wrong with them run its course. And to go even further than that, stop changing the tank around. adding new gravel, taking it out, removing everything in the tank, etc. is al putting more stress on the fish. You are going to overlove that fish to death. You need a heater, obviously, and good husbandry practices and that will help you tremendously.
I wonder what the tank is filtered with? Is there sufficient water flow? Do you have enough surface agitation to provide an ample amount of oxygen transfer? These are all questions that should be answered in conjunction with your parameters before you even begin to think about medicines. If its ick and you know it is 100% then treat that, flukes as well, otherwise stop, period. Good luck!
