Any ray shot to the chest doesn't have a happy ending. I've heard the barb was around 1o inches. Depending on age of the ray, that could be true. Just with size alone, the wound would be fatal.
From what we've heard, he approached from behind, swimming along the bottom. That means the angle of the tail would have pushed it into his chest right below the sternum, straight into his heart. This is both the easiest way to swim the bottom and the most risky. It leaves your body cavity exposed. It really surprised me to hear he was swimming that way along the reef.
