Your biggest mistake

got drunk. blacked out. tried to find the bathroom. tripped on a trunk on the floor. fist found a 10 gallon tank. whoops
 
Didn't bother to read up on using driftwood from lakes before tossing it in my 55 with my jd and then going on vacation. I could smell the rotting log + fish as soon as I walked in the front door.
 
Decided that rinsing gravel before putting it in the tank was not important.
 
I grabbed the flake food instead of the freeze dried blood worms(which ha a shaker lid) dumped the entire can of flakes into my 55 gal. I was cleaning that mess for two days. But I'm very new at this so give me more time I'm sure I can top that.

Falling down the basement stairs in the dark at 4am after unexpectedly stepping on an escaped crayfish.
I had a brief vision of a rabid monster movie rat gnawing on my bare foot, jerked back and missed the step.
Neither I nor my intermission snacks made it entirely intact to the comfort of my Lazy-Boy and the remainder of the monster movie marathon.
The phrase "ass over tea kettle" may have been coined for just such an occasion.

Did the same thing when I first started. The heater cracked and cooked fish.
 
My very first fish I every owned;

I had a South American Puffer that needed it's teeth clipped every 4 months. I fed him clams, crawdads and home grown snails, but still the clipping needed doing. So every 4 months I put One drop of clove oil in 3 cups of tank water and put him under to do it. This went on for years. Then one day I had a brain fart and accidentally put three drops of clove oil for one cup of tank water. He died. I was devastated. Lesson learned.
