NEVER GO ON HOLIDAY RIGHT AFTER A NEW TANK SETUP! I had a 4 foot tank that I overstocked with high-end Goldfish. I went for a 2 week holiday 2 days after I introduced my last fish and let my dad look after them. During that time, my nephew visited regularly, overfed my overstocked tank, filter clogged (assumed from a dead fish my dad did not notice). I came home to an empty tank with $600 literally down the toilet. :(

Had a 75g needed to treat it with meds. My husband took the carbon out of the filter, put it in a zip lock bag and put it in the fridge. 3 days later, took the carbon out of the fridge and put it back in the filter... woke up the next morning to dead and dying fish. Lost over $400 in plecos. Fish were bleeding the ammonia was so bad.

Put my pbass/geo growout tank in the kitchen rather than my basement. My daughter is in her terrible twos and it just started lol... :(

woke up one morning to my three year old "fishing " in my 75 gallon. looked more like swimming to me.. water all over my floor

When I had just started in the hobby I bought a new tank, dumped the fish in without cycling, and neglected to get a proper top. I woke up the next morning to my cats eating fish on the floor. Don't know why I didn't expect that outcome.

mine were two this year, first i put a flower-horn in with my Oscars and the flower-horn beat the crap out of a few of them. too bad too because he was a nice looking fish. the second mistake was an expensive one, i purchased a fish off ebay that i was so excited to get it i was at the mailbox waiting on the mail man patiently. it was a rtcxtsn that i paid twenty bucks for and an additional fifteen to ship it overnight. got the fish so excited he looked a little smaller then expected, put him in with the other Oscars and cats and watched as one of the oscars sucked the whole fish in his mouth... gone thirty five dollar snack! i learned the hard way if it looks like it could be eaten it will be.

i used a garden hose off the sink to fill the 55 did not know that the hose came off the sink until i realized i was loosing water, flooded the kitchen badly and lost two fish i found under the towels when everything was dry. what a ride they must have had at about a hundred threw the hose then get stepped on by me and the kids trying to dry it all up. ooops
